Enrollment fee includes all scientific sessions, course syllabus, Friday evening reception at the Charleston Harbor Resort & Marina, continental breakfasts, lunch and a certificate of course completion. Attendance is on a first-come, first-serve basis, and limited to 200 participants due to the interactive format. Registration cancellations must be received in writing before February 1, 2010. Your paid tuition, less a $50.00 administrative fee will be returned. No refunds will be granted after February 1, 2010.

Casual business attire. Some may find a sweater comfortable for symposium sessions.

Accreditation
Physician Accreditation:
The South Carolina Medical Association designates this educational activity for a maximum of 13.5 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Nursing Accreditation:
Village Fertility Pharmacy is an approved provider of continuing nursing education by the California Board of Nursing, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

Provider approved by the California Board of Registered Nursing, Provider #CEP 15206, for 16 contact hours.
Provider approval expires October 31, 2010.

Additional Accreditation:
Nurses who choose to participate in the Donor Eggs 101 Mini Symposium will receive an additional 3 AMA PRA Category 1 credits™ or 3 contact hours.

Accommodations
The symposium is being held at the Charleston Place Hotel located in historic, downtown Charleston. A block of rooms has been reserved and will be held until February 1, 2010 at a special conference rate of $259.00 single/double occupancy, plus tax, per night. To reserve your accommodations at the discounted rate, contact the Charleston Place Hotel at 1-843-722-4900 or toll-free at 1-800-831-3490 and mention the group name “The ART of Donor Oocytes.” Hotel reservations can also be made at groupres@charlestonplace.com. Reservations can be confirmed by sending a deposit for one night’s room charge or by credit card. A 72-hour cancellation notice must be given to avoid penalty. Reservations received after February 1, 2010 are subject o availability. Deposits are fully refundable if the reservation is cancelled 72-hours prior to arrival date.
