Enrollment fee includes all scientific sessions, course syllabus, Friday evening reception, continental breakfasts, lunch and a certificate of course completion. Attendance is on a first-come, first-serve basis, and limited to 200 participants due to the interactive format. Registration cancellations must be received in writing before April 30, 2008. Your paid tuition, less a $50 administrative fee, will be returned. No refunds will be granted after April 30, 2008.

Casual business attire. Some may find a sweater comfortable for symposium sessions.

Physician Accreditation:
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of Medical Education Resources and the Art of Donor Oocytes and Third Party Reproduction LLC. Medical Education Resources is accredited by the ACCME to provide continuing medical education for physicians.

Credit Designation:
Medical Education Resources designates this educational activity for a maximum of 16 AMA PRA Category 1 credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Nursing Accreditation:
Medical Education Resources is an approved provider of continuing nursing education by the Colorado Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

This CE activity provides 16 contact hours. Provider approval expires July 31, 2010.

Participants who choose to attend the Donor Eggs 101 Mini Symposium will receive an additional 3 AMA PRA Category 1 credits™ or 3 contact hours.

The symposium is being held at the Charleston Place Hotel located in historic, downtown Charleston. A block of rooms has been reserved and will be held until May 5, 2008 at a special conference rate of $320 single/double occupancy, plus tax. Reservations can be confirmed by sending a deposit for one night’s room charge or by credit card. A 72-hour cancellation notice must be given to avoid penalty. Reservations received after May 5, 2008 are subject o availability. Deposits are fully refundable if the reservation is cancelled 72-hours prior to arrival date.
